How to Prepare for Your First Appointment
A little preparation helps ensure your first visit goes smoothly and allows your physician to make the most of your time together.
Get a Referral
All new patient appointments require a physician referral. Ask your primary care physician, specialist, or hospital provider to send a referral to our office. Once we receive the referral, our scheduling team will contact you to verify insurance and schedule your visit. We do not accept self-referrals or walk-ins, including for PrEP, HIV testing, or travel medicine.
Verify Insurance
Contact our office or your insurance company to verify that we are in network with your plan. Our staff will work with your insurance to obtain prior authorization when required.
Gather Documents
Collect your photo ID, insurance card, referral paperwork if applicable, a list of all current medications, relevant medical records or imaging, and any test results from other providers.
Arrive Early
Plan to arrive 15 minutes before your scheduled appointment to complete registration and paperwork. Bring a list of questions you want to discuss with your physician.
Appointment Checklist
- Photo identification (driver's license, state ID, or passport)
- Insurance card (both front and back if applicable)
- Referral paperwork from your physician if applicable
- Complete medication list including prescription drugs, over the counter medications, vitamins, and supplements
- Medical records related to your condition, including recent lab results, imaging studies, or hospital discharge summaries
- List of allergies to medications, foods, or other substances
- Questions you want to ask your physician during the visit
- Payment for any copays or deductibles if applicable

Records, Billing, and Alternative Provider Locations
The resources below are for existing patients, former patients, and anyone seeking care for services we no longer provide.
How to Request Your Medical Records
You have the right to obtain a copy of your medical records under HIPAA. To request your records:
- Call our office at (479) 444-6522 to begin a records request.
- We will send you a signed HIPAA-compliant records release authorization form by mail, email, or fax. Complete and sign the form.
- Return the completed form to our office. Records are typically released within 10 business days of receipt of the signed authorization.
- Records can be sent electronically to your new provider, by fax, or as printed copies for you to pick up.
If you need records released urgently (for example, an upcoming appointment with a new provider), let our staff know when you call so we can prioritize the request.

For questions about a statement, charges on your account, insurance billing, or to make a payment, please contact our office:

- Pay by phone: Our billing team accepts credit and debit card payments by phone during business hours.
- Payment plans: If you cannot pay your balance in full, ask about payment plan options.
Please have your patient account number or date of birth ready when calling so we can locate your account quickly.
